Request for Modification/Improvement Form

• All modifications/improvements to the exterior of the home must be approved by the Board of Directors or Architectural Review Committee (ARC) prior to starting the project, except for an emergency to property or life.  A $1500.00 fine/occurrence may be imposed if there is a failure to submit a request.
• A member from the Board/ARC may contact the homeowner within two (2) business days for more information or clarification, if necessary.
• Include drawings to scale, pictures, surveys, and permits, if necessary.
• Obtain permits and approvals required by DeKalb County. Post as required.
• Ensure a dumpster/receptacle is onsite PRIOR to beginning work. ALL trash and debris must be disposed of properly at the end of each work day.
